Management
and Evaluation is a preventative program designed to reduce the risk
of hospitalization, monitor medical needs, help promote recovery
and ensure the patient's medical safety. Anyone at high risk
for hospitalization, experiencing changes in a chronic medical
condition or is in need of supervision by a registered nurse,
may qualify for this Medicare approved program.

- Compromised
mental status
- History
of re-hospitalization
- Cultural
or language barriers to traditional care
- Home
setting is unsafe or inaccessible
- Caregiver
has limitations or dysfunctional
- Relies
on multiple or complex equipment
The
ME team will evaluate medication compliance, nutrition/hydration
status, functional status and increased risk for potential complications. |
